What to bring to your free consultation

In order to better help you with the insurance process for your cranial prosthesis there are some things you will need to bring with you to your free consultation.

• Drivers License or Valid ID
• Your insurance card
• A prescription from your physician for a "cranial prosthesis"
• A letter of necessity from you doctor explaining that your hair loss is not simply a cosmetic issue but an emotional one as well.
